Membership Form (not PayPal)

This membership form is to be used by those members who've paid their dues already. Either you've paid a Board Member in cash, or you are a Lifetime member, etc. In either case, please fill out the form below as best you can and click "Submit Form". The form tells us who you are, is the release of liability, and is the agreement to the SDF Code of Ethics. It is to be filled out annually by each member. Clicking "Submit Form" at the bottom will redirect you to PayPal, but you can exit out at that point (after submitting the form) since you won't be paying with PayPal. Thank you.
